Practice Area
If you have moved to the practice area (we do not accept applications to register from people outwith our Practice area), call in at the surgery between 08:00 and 17:30 Monday to Friday to complete a registration form for each person. Do not wait until you are in urgent medical need.
Remember that if you leave the area and the practice, you must let us know and register with another GP practice. Do not wait until you feel ill and need to see a doctor as, for example, you may need health reviews or certificates for insurance.
Registering with a GP practice
First check that you live within the boundaries of the Practice Area. If you do then please download and print the two forms linked below, fill them in and then bring them along to the Surgery.
You will need to be registered before you can make an appointment.
It can sometimes take a while for new patient's medical records to come to us from their old Surgery. So please include as much medical history in our patient questionnaire.

These forms can be completed then emailed to the practice at craigvinean.tayside@nhs.scot. Just type your name where signatures are asked for.
When you register with us, it’s recommended that you provide information to confirm your:
- identity
- address
This means you can be matched to your existing NHS records. You shouldn’t be delayed or refused registration at a GP practice if you can’t provide identification or proof of address.
Anybody in Scotland can access primary care services at a GP practice without charge.
Registering as a Temporary Resident
You can only register with a GP practice if you’re staying in the area for more than 3 months. If you’re not, you may still be treated but as a Temporary Resident.
You can also use the Access to Healthcare. This is a GP registration card to help you register with a GP Practice. The card gives information on your rights to accessing healthcare. You can find the card on the Health Literacy Place website.

Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
